Clinical signs of chocolate poisoning
Signs usually appear 6-12 hours after ingestion, and can continue for up to 72 hours because of theobromine's long half-life. If your dog has just swallowed it and is still asymptomatic, do not wait until symptoms appear to act, the sooner the decontamination, the better the outcome.
Early signs (first 2-6 hours)

Pancreatitis (inflammation of the pancreas) is also a secondary risk because of the high fat content in many chocolate products, its signs are repeated vomiting, severe abdominal pain, a "praying" position (front lowered, rear raised), and it can appear 24-72 hours after ingestion.
First aid when a dog swallows chocolate
What you MUST do
- Call a 24-hour vet clinic IMMEDIATELY, even before the dog shows symptoms. The most effective treatment is given within the first 2 hours after ingestion, before theobromine is maximally absorbed in the gut.
- Note down the important information before calling: the type of chocolate (brand, cocoa percentage if listed on the packaging, the more concentrated, the more dangerous), the estimated amount swallowed (check the remaining packaging to calculate how much is missing), the dog's weight, the approximate time it was eaten, and any symptoms present.
- Keep the packaging and any leftover chocolate, bring it to the clinic so the vet can check the content and estimate the dose.
- Take the dog to the 24-hour clinic if the vet advises, usually for doses above the mild threshold, or a small dog with a dark/baking chocolate ingestion, the vet will ask you to come in immediately for decontamination.

What you must NOT do
- DO NOT induce vomiting yourself at home by any method (salt, hydrogen peroxide, baking soda) without a vet's instruction, many home methods are actually dangerous:
- Table salt can cause sodium poisoning (hypernatremia), which itself triggers seizures

- DO NOT give milk to "neutralize" it, a myth. Milk does not neutralize theobromine, and many dogs are lactose intolerant → more diarrhea.

- DO NOT give coffee or a "counter-stimulant", caffeine actually makes the poisoning worse.
- DO NOT wait to "see whether symptoms appear", if symptoms have already appeared, the window for effective decontamination has passed. Quick action in the first hours is the key to a good outcome.
- DO NOT try to give activated charcoal from a human pharmacy without a vet-determined dose, the formulation and dose are different; the wrong dose is ineffective or triggers complications.
When this is a 24-hour-clinic emergency (not a house call)
Chocolate poisoning (and poisoning in general) is a scenario where a house call is not enough. Effective decontamination treatment requires:
- Inducing vomiting with an injectable drug (apomorphine) at a specific dose and with the vet's technique
- Oral activated charcoal at the correct dose (often needing repetition every 4-8 hours)
- IV fluid therapy for forced diuresis to help eliminate theobromine via the kidneys
- Continuous ECG monitoring if there is an arrhythmia
- Anti-arrhythmic drugs (lidocaine, beta-blocker) IV if needed
- Anticonvulsant drugs (diazepam, midazolam, levetiracetam) IV if seizures occur
- Hospitalization of at least 24-72 hours for monitoring because of theobromine's long half-life

- Inducing vomiting is usually no longer effective because the chocolate has already passed the stomach, the focus shifts to supportive care
- Multi-dose activated charcoal is still used to interrupt enterohepatic recirculation
- Aggressive IV fluids with forced diuresis
- Continuous ECG monitoring for arrhythmia
- Anti-arrhythmic drugs if there are PVCs (premature ventricular contractions) or ventricular tachycardia, lidocaine IV first-line
- Anticonvulsant drugs if seizures occur, diazepam IV/rectal as first-line, levetiracetam IV for maintenance
- Methocarbamol if there are significant muscle tremors
- Hospitalization with monitoring for up to 48-72 hours (until enough theobromine is eliminated)
The prognosis depends on the dose swallowed, the timing of intervention, and the dog's initial condition. A dog brought in within the first 2 hours and decontaminated quickly usually has a good outcome. A dog already having seizures or a severe arrhythmia on arrival has a more guarded prognosis.
Other household toxins that are just as dangerous
Some other toxins that are also emergencies if a dog swallows them:
Grapes and raisins (grape/raisin toxicity)
- The mechanism of toxicity is not fully understood, but it can cause acute kidney failure
- The toxic dose varies greatly between individual dogs, some dogs are affected by a small amount, some tolerate more. There is no reliable "safe dose"
- This includes rum balls, brownies with raisins, raisin bagels, grape juice

- Store chocolate and candy where the dog cannot reach, a closed cabinet, an upper cabinet, not on the dining table/coffee table
- A guest's bag with candy inside = a hazard, especially a bag placed on the sofa or floor. Brief guests about your home dog.
- A kitchen trash bin with a lid, dogs often steal from the trash (used chocolate wrappers, cake leftovers)
- Holidays are high-risk, Christmas (gift chocolate), Eid al-Fitr (cakes with chocolate and raisins), Halloween (children's candy), Valentine's (boxed chocolate). Brief the whole family.

My dog ate one small piece of milk chocolate, is it an immediate emergency?
It depends on the dog's weight. For a dog >20 kg, one small piece of milk chocolate is probably not fatal, but still call a 24-hour clinic for a consultation, do not assume on your own. For a small dog (<5 kg), one piece of milk chocolate can already trigger symptoms. For a small dog that eats dark chocolate or baking chocolate, always an emergency.
My dog vomited on its own after eating chocolate, so is it safe now?
Not automatically. Spontaneous vomiting sometimes expels part of the chocolate but not all of it, the remainder still in the stomach and gut will still be absorbed. Homemade brownies with raisins, a double problem?
